The first 2 pretreatment photos shows the cumulative wear and tear that normally occurs and becomes more and more evident as restorative procedures are not immediately pursued when the need arises.The patient has damaged and broken denture teeth, infected gums, missing teeth and porcelain crown failures where decay is evident at the crown margins. The patient felt his smile and jawbone relationships were beginning to collapse due to the wearing down of his teeth. This process is commonly known as loss of vertical dimension. The adjacent closeup photo reveals several broken, chipped and jagged biting surfaces on several natural and denture teeth.As bites begin to collapse, physical relationships between the biting surfaces of the upper and lower teeth become unbalanced and can cause accelerated damage to small tooth structures due to the increasing malalignment of biting (incisal edge) surfaces. Over time, the bite can collapse quite rapidly, and if left untreated for too long, can create tmj and tmd problems, leading to a variety of unusual pain symptoms and even noises caused by unnatural jaw movements. 
The adjacent post treatment photo shows the dramatic changes in Vertical Dimension (the new total height achieved with longer teeth on the upper and lower jaws). A balanced combination of new denture materials, new crowns and veneers reestablished the physical relationships needed for having a normal bite for this patient. 
Extensive use of articulation studies and waxup modeling assured that all functional and cosmetic goals would be achieved as defined by the patient.The patient's face no longer shows a squared-off appearance, which is typical for collapsing smiles. Rather, the face has become longer, giving a more youthful appearance. The patient was quite pleased with the outcome. The patient reported a renewed sense of self confidence that affected for his occupation and social activities.
